How Care Works and Patient Forms
Step 1: Submit a Confidential Consultation Request
Start by completing the consultation request form. This helps Ambire Health, Inc. understand your needs, location, preferred appointment type, and the type of support you are seeking.
Step 2: Initial Review
Your information will be reviewed to determine whether Ambire Health, Inc. may be an appropriate fit. Some patients may need emergency care, primary care, specialty care, inpatient services, or a higher level of treatment.
Step 3: Appointment Scheduling
If Ambire Health, Inc. appears appropriate for your private care needs, an appointment may be scheduled by telehealth or private in-home consultation.
Step 4: Individualized Care Planning
During your consultation, your medication history, current concerns, goals, and care needs will be reviewed. The focus is on safety, stability, dignity, and practical next steps.

Notice of Privacy and Practices
Ambire Health, Inc. is built around an appointment-only private care model. The practice avoids the feel of a high-volume clinic and does not operate from a public walk-in location. Patients can expect care that emphasizes:
- Privacy
- Respect
- Patient Dignity
- Clinical Discretion
- Medication Safety
- Clear Communication
- Individualized Planning
Financial Policy
Ambire Health, Inc. uses a private direct-care model. This allows the practice to focus on individualized attention, continuity, and direct access to clinicians. Patients should expect a clear discussion of fees, services, and payment expectations before care is established.
Insurance billing, prior authorizations, medication coverage, lab costs, imaging costs, pharmacy costs, and outside specialist fees may vary and may not be included in Ambire Health, Inc. service fees.

Private In-Person Consultations
For Ohio patients who need or prefer a more personal care setting, Ambire Health, Inc. may provide private in-person consultations by appointment. In-person consultation may be appropriate when privacy, mobility, transportation, or personal circumstances make traditional clinic-based care difficult or undesirable. Please note that all in-person appointments should be scheduled in advance, as we do not provide walk-in or on-demand urgent care services.
Telehealth Appointments
Telehealth allows patients to receive care from a private location via a secure virtual visit. Telehealth may be appropriate for medication-assisted treatment follow-up, recovery support, medication review, care planning, and ongoing clinical guidance when clinically appropriate. Some services or situations may require additional evaluation, documentation, lab testing, pharmacy coordination, or referral to another level of care.

Take the First Step
If you are seeking private care and support for recovery, medication-assisted treatment, medication transition, or complex medication management, contact Ambire Health, Inc. today to begin with a confidential consultation.
Important Note: This website is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.

How soon can I schedule my discovery call?
Many discovery calls can be scheduled within a few business days, depending on availability. We’ll work with you to find a convenient appointment time.
How are appointments conducted?
Most appointments are conducted through secure telehealth. Ohio patients may also be eligible for limited in-home visits in select communities when appropriate. We’ll discuss the best option during your discovery call.
What happens after my initial evaluation?
If we decide to work together, we’ll develop an individualized care plan based on your goals and clinical needs. Follow-up visits, medication management, secure messaging, and ongoing support are provided through your quarterly membership.
Do you prescribe controlled medications?
When clinically appropriate and consistent with safe prescribing practices, controlled medications may be prescribed as part of an individualized treatment plan. Every recommendation is based on a careful medical evaluation, current guidelines, and ongoing monitoring.
What if I need emergency or crisis care?
Ambire Health is not an emergency or urgent care practice. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department. Patients experiencing a mental health crisis should contact 988 or seek immediate emergency assistance.
